Strabismus treatment in children

Provides interesting information from the long history of the doctrine of binocular vision, strabismus, the clinical course of the disease.
Based on the literature data and own experience of work in the first in the USSR specialized summer camp of sanatorium type, the author convincingly proves the expediency of implementation of the rehabilitation of children with strabismus and amblyopia.

Preface

Concomitant strabismus is one of the most common forms of pathology of vision in children. It occurs in almost 3%. children. In recent years, this percentage gradually declining, especially in Moscow (1,5%), which uses the method of mandatory testing of clinical refraction and spectacle correction in children one year of age.
In the USSR developed and introduced in a wide practice of evidence-based treatment of strabismus and amblyopia. Its application allowed us to achieve favorable results in 85% of cases. However, almost half of the treated children are still unable to achieve the restoration of binocular vision.
Until the last decade of conservative treatment of children with strabismus was conducted in the outpatient setting. Since the 60-ies in the major cities of preschool children were treated at the eye spectatcular, and in Moscow and Leningrad in the children's eye health resorts. Meanwhile, a significant proportion of school-age children with the most severe disorders of binocular vision that require the use of a long and sometimes repeated courses of treatment, deprived of such opportunities due to the lack of free time. This leads to partial loss of the already reached school age results. Only about 30% of children in the process of studies regularly undergo treatment in the offices of protection of sight of children, created in many cities of the country. In rural areas the situation is even more complicated.
Certain difficulties in achieving a functional cure children with strabismus due to the fact that still poorly developed methods not only recover, but to determine the depth of binocular vision in children of different age. Known obstacle is the fact that the recommendations on treatment of children with strabismus, developed by the Department of protection of children's vision of the Moscow research Institute of eye diseases named. Helmholtz under direction of the honored worker of science, Professor E. S. Avetisova, not always quickly being introduced in a wide practice. All this prompted us to undertake the search for more efficient methods of strabismus treatment in children of school age.
We believe that the optimal factors for sound management of school children with strabismus can be collectivity of treatment, continuity and intensity of his, objectification control testimony to the individualized treatment and estimation of its efficiency, organization of treatment in complex with health-building activities and recreation. These factors can be realized in period of summer holidays.
In 1967, for the first time in the USSR in the Crimea was established pioneer camp of sanatorium type, where the treatment of children with strabismus and amblyopia was combined with recreation. From 1967 to 1973 in the camp under our supervision there were 806 children. Results of observations became the basis for writing this monograph. It also presents data on methods of determining profound vision device own modifications, which is acknowledged as innovative invention branch value (survey Instrument threshold of binocular vision - PHBS). This device allowed to obtain new data on the state of deep binocular vision in healthy children of school age and at a similar age group of children in the final stages of treatment of strabismus. On the basis of initial data about the state of the visual analyzer, his sensory and motor functions were defined indications for the most oriented methods of treatment.
The results of the conducted researches allowed to present convincing data to the health authorities, public education and the advice of trade unions that rehabilitation of children with strabismus and amblyopia to ensure the continuity of treatment it is advisable to carry out in the conditions of specialized pioneer camp.
Thus, the rationality of the widespread introduction of this experience.
