Organization of narcological assistance

Currently, the country formed a special drug service that appears as a step specialized system of preventive and curative care to persons suffering from chronic alcoholism, narcomania and toxicomania.
The main link in the organization of narcological assistance to the population is narcological dispensary (Republican, regional, regional, city), included into the list of health care institutions by order of the Minister of health of the USSR on February 5, 1976, In the structure of narcological dispensary include both ambulatory units (district narcological rooms, treatment rooms, and medical treatment points, at the industrial enterprises, and stationary and semi-stationary (day hospitals and night dispensaries in industrial, construction, transport enterprises) narcological Department. In narcological dispensary are also special rooms for teenagers, of the intoxication, psychotherapeutic, and various support and Advisory unit. For example, the Tomsk oblast narcological dispensary is composed of various laboratories, including psychological, day hospital, the doctors of different specialties, psycho-hygienic office for teenagers, legal advice, specialized CWC, the Department of functional diagnostics, organizational-methodical Department.
Narcological dispensary is the organizational-methodical centre of struggle against drunkenness and alcoholism.
In accordance with the "Regulation on narcological dispensary", approved by the Ministry of health of the USSR on August 20, 1976, narcological dispensary on its territory of a service should implement:
- the identification and registration of patients with alcoholism and the provision of specialized care (inpatient and outpatient);
- study of the incidence of alcoholism and analysis of the effectiveness of therapeutic and preventive care;
- psycho-hygienic and preventive work within dispensary, and out of it (at industrial enterprises, in educational institutions and others);
- medical examination of persons aimed to compulsory treatment for alcoholism, as well as temporary disability examination and examination of alcoholic intoxication;
- consultative and methodological assistance to drug treatment rooms, a part of outpatient, psycho-neurological institutions, health units, and narcological points at industrial enterprises;
- organization and holding events on specialization and advanced training of doctors and nurses;
- participation in the preventive measures to combat alcoholism in conjunction with other departments, organizations and institutions.
Drug service is based on a district principle. The organizer of drug treatment is a district psychiatrist-narcologist, which conducts all therapeutic, preventive and social activities on the site, carrying out the necessary link with organizations and institutions in the service area. The position of physicians - catchment area of addiction psychiatrists are established at the rate of 0.2 posts per 10,000 workers in industrial, construction and transport enterprises and organizations and 0.1 posts per 10,000 inhabitants the rest of the adult population of the city and other settlements, the inhabitants of which is attached to the dispensary for direct ambulatory care. In rural areas the posts of district psychiatrist-narcologist are also depending on population size, but not less than 0.5 positions in the district.
Drug offices, which employ the district narcologist, should be as close to the population. Therefore, they should be created not only in the premises of the center for the population of the adjacent areas), but in the medical institutions (polyclinics, medico-sanitary parts) of the service area, geographically remote from the center.
