In recent years the number of patients with gout increased dramatically. Often this disease affects people suffering from obesity. The fact that they often upset the regulation of activity of the liver and kidney, particularly their enzymatic and metabolic functions, which is the accumulation in the body of a large number of intermediate products exchange. In the result accumulated beta-lipoproteins, cholesterol, uric acid salts and many other intermediate products exchange. If the first contribute to the development of atherosclerosis of the coronary, cerebral and main vessels, the accumulation of uric acid salts and their accumulation in different tissues of the body contribute to the emergence of a gouty attack.
How does this process and how is it manifested? Deep changes that occur in the body during a gout attack, unclear and require further at the cellular and molecular level, study. However formulate certain views on the development of this complex process is now possible.
Clinical observations and special radiological studies show that in patients with gout are violated absorption and renal excretory function of hepatocytes (liver cells). How gouty attack occurs? Observations show that such an attack usually appears within a few hours after a heavy dinner, when the patient has eaten a lot of proteins and fats, and alcohol. This hearty lunch is, figuratively speaking, the last straw that broke the internal environment of the body's proteins. It is assumed that overflow of liver cells by purines (breakdown products of proteins) leads to the accumulation in the blood uric acid, which due to lowering of the excretory function of the kidneys accumulate in the body and converted into uric acid salts, postponed in particular, if I may say so, storerooms of the human body. Such pantries are synovial membrane, especially the metatarsophalangeal joints of the big toes, elbow, ankle and knee joints, and vaginal of tendons of muscles of the ear and coronary vessels.
Uric acid salts into the antigens, probably because these salts damage the cell structure and the resulting debris in combination with themselves salts acquire these properties. Antigens, in turn, cause the formation of antibodies, their meeting is carried out on the microcirculatory level; as a result of these complex changes occur an acute attack of aseptic inflammation in the joint. The resulting process is characterized by the sudden appearance of a violent pain in the big toe, knee, ankle, elbow, the patient feels as if these joints are covered with a hot iron.
Gout is not comparable to the pain of any other origin. Within a few hours appear one after the other additional signs of acute inflammation: redness of the skin, increasing its temperature, swelling of soft tissues, infringement of function of the affected joint. Increases the number of cells in the blood, is accelerating the sed rate.
Such signs of inflammation can occur in any other inflammatory processes, therefore, essential to clarify the diagnosis is determination of uric acid concentration in the serum. To conduct this research in a patient previously, within three days, exclude from dietary protein. Then by a special method of determination of the amount of uric acid. Increased levels of this acid in the blood in two and even three times is irrefutable proof of gouty process.

Often gout occurs inflammation in the mucous bag elbow joint. Bag, which usually cannot be felt, i.e. the size of chicken eggs; swelling painful movement in the joint significantly limited (Fig. 8).

The disease takes chronic prone to relapses over. It continues throughout life. Along with this gouty process can affect the ear (Fig. 9), hands and other parts of the body.
