The number of patients with diseases of heart and vessels is increasing every year. It grows at the expense not only of acute illnesses, but also transition separate forms in the chronic stage, often receiving relapsing course. The first place among this group of diseases is coronary heart disease.
The concept of "ischemia" means a lack of blood supply, in particular, certain areas of the heart muscle. Of course, for understanding this process and its consequences need to determine in what circumstances and under what conditions occurs ischemia. Of considerable importance in the development of ischemia has a spasm of coronary vessels (i.e. vessels that supply blood to the heart), which leads to insufficient blood circulation. This spasm, in turn, reduces the delivery of oxygen to muscle cells (myocytes), causes a disturbance of metabolism in them. In violation of the exchange collects the products that irritate found in muscle end of nerve fibers and cause heart pain. Pain reinforce spasm of small branches of the coronary arteries, which is still more disturbs the blood supply of one or another section of the myocardium.
So, thus, a vicious circle in capillaries spasm affects the delivery of oxygen to the cells, these cells begin to "choke", and then the pain is getting stronger, and they, in turn, enhance spasm. Ischemia develops as a result of atherosclerosis of the coronary arteries, leading to a narrowing of the clearance and occurrence of strokes. They arise when the imbalance between demand and supply of oxygen. These attacks, in turn, lead to ischemia of separate areas of the myocardium.
Thus, three factors - atherosclerosis, the narrowing of the coronary arteries and their spasm play a leading role in the onset and progression of coronary heart disease.
Cause spasm of the coronary arteries are often negative emotions that reflex by not only lead to the reduction of the lumen of the coronary arteries, but also to spasm in the small vessels of the heart, causing myocardial ischemia. The violation of circulation (i.e. the promotion of blood between muscle cells) negative effect on the trophic processes in myocytes.
It should, however, be borne in mind that people with high emotional arousal such attacks cause the fear of death. When the mismatch between demand and supply of the heart muscle in energy materials, and especially in oxygen, the pain is unbearable and on this background may occur myocardial infarction.
True, in the heart are the compensatory mechanisms, able to prevent the death of separate areas of the myocardium. But if there stenoziruyuschego atherosclerosis of coronary vessels, these mechanisms do not always work, and as a result increasing ischemia, a focal or common myocardial infarction.
With coronary heart disease suffers from "pumping function of the heart, it pumps into the arterial system smaller than normal, the amount of blood, this increases the amount of blood in the veins. In case of an emotional or physical stress activates the sympathetic-adrenal system and then in the cardiac muscle collects the products of activity of this system is catecholamines (epinephrine, norepinephrine, dofamin), for oxidation of which requires an increased amount of oxygen. However, in patients with ischemic heart disease into force of atherosclerosis and metabolic disorders redox processes worse, ischemic and trophic changes in myocytes are increasing.
